In order to minimize health risks to our UHS community, all visitors will have their temperatures checked and be asked screening questions at the reception desk. Thank you.

Those who do not have all completed documents including previous years' grades will not be enrolled and will be asked to obtain the required information and reschedule. We are not able to make copies of provided documents, sorry for this inconvenience.

Student Registration


Those who do not have all completed documents including previous years' grades will not be enrolled and will be asked to obtain the required information and reschedule. 

  • English - Student Registration Packet

  • Spanish - Formulario para matricula de estudiante
  • Creole - Pake Enskripsyon Elèv
  • Arabic - حزمة تسجيل الطلاب

Required documentation:

*Student Birth Certificate or Passport

*Immunization on a Form 680, which can be obtained at the

Orange County Health Department: 

*Proof of physical examination

 by a U.S. doctor within a year of enrollment (first day of entry at school).

*Transcript Grades

*Verification of Special Education Information(if applicable)

Current, IEP OR 504 Plan

*Verification of your Domicile in Orange County (one of the following)

  1. Current Homestead Exemption Card
  2. Property Tax or Signed Settlement Statement
  3. Current Signed Lease
    Verification of Residence Information

*Parent PhotoID - Driver's Licence, State ID or Passport

*Bring the completed registration packet and required documentation to your school. 

*Students can only be withdrawn by Parents or Legal Guardians. 

When withdrawing please make sure to bring the following:

*Student Laptop
*Student Number
*Parent Identification 
*Transferring School Information
